GENERAL TERMS OF SERVICE: ----------------------------------------------------------------------------------------- GENERAL LICENSING: Personal Touch Catering is licensed by the Health Department, the City of Frankfort and Franklin County. We have a centrally located catering kitchen in the city of Frankfort and invite you to come by any time to see our facilities. BEVERAGE LICENSING: Personal Touch Catering is licensed by the Alcohol and Beverage Control department and the city of Frankfort. We are able to provide you with a complete beverage service. As a licensee we are responsible for complying with the laws regarding these regulations. SALES TAX: Personal Touch will add a 6% sales tax charge to all food and beverage items. Taxable items include supplies, rentals, food and any item priced inclusively. Labor is tax exempt only if priced separately. If you are tax exempt, appropriate documentation is required. Tax Exempt: Clients and or organizations that are tax exempt are required to provide a certificate on or before the date of the event. DEPOSIT: A deposit of 100.00 is required to book the requested date unless you are with a state agency. This deposit reserves your date and is highly recommended, especially for weekends. The deposit is non-refundable but will be applied to the balance of your invoice. GUEST COUNT: Fourteen days (or an agreed upon date) before the event, you will provide Personal Touch will a final guest count. After that date you may increase the number of guests but not decrease. FOOD AND BEVERAGE ESTIMATES: Total costs vary according to menu selection and type of event. All food, beverage and supplies will be provided by Personal Touch unless other arrangements are agreed upon. Alcohol may only be served by a Personal Touch Bartender. When choosing two entree's, both entrees are priced at the higher priced item. Personal Touch will be prepared to serve up to 5 percent above the guarantee for buffets but the five percent is NOT guaranteed. Exact guest counts are required for some menu items. TASTINGS: We do not offer pre-booking tastings but instead will contact when you have reserved a date with us to come to other events for an "event" private tasting. This allows a broader taste of the menu items and a showcase presentation of our food in a realistic event setting. STAFFING: Average time allowed in addition to the actual duration of your event for setup, breakdown and cleanup is 3 hours. Staffing charges are $15.00 per hour per wait or culinary person required for your event. We suggest one wait person per 10 - 20 guests for a seated event, one wait person per 20 - 50 guests for a buffet event. A five hour minimum charge is required for a staffed event. ADDITIONAL CHARGED SERVICES: Personal Touch provides these additional services. Events and pricing differ depending on the number of additional services required. * Rental China unpacking, cleaning and repacking - (China Services .35 - 1.00 per person) * Upstairs delivery of rental items - (Usually 1.00 per table or chair, flat fees for china) * Delivery - (Location specific) * Pick up on rental items * Set up and or strike of rentals * Dining table decor or placement of furnished decor Paper disposable products are included in the pricing of many of our items. Disposable plastic items are charged at .25 - 1.00 per person and china rentals are priced per piece. Please ask about these options for your event. If a return trip is required to pick up equipment outside of the Frankfort city limits, a service fee is added and a deposit may be required. CAKE CUTTING AND SERVICE: Our professional servers will be pleased to cut your wedding or grooms cake at no charge. AUTOMATIC GRATUITY: A gratuity amounting to 12% will be automatically added to all corporate and social events. I accept:
